* The hourly rate for this role is up to $19 per hour.

Position Summary

The Member Support Team is responsible for daily front office operations including performing a wide range of duties both administratively and in support of clinical staff. The Member Support Staff is responsible for engaging and assisting our teams and our members in pursuit of our mission to ensure quality services are delivered to improve the health and wellness of our members.

Specific Responsibilities

• Provide exemplary customer service
• Maintain professional demeanor
• Check members in as directed and update any relevant demographic information including but not limited to address, phone number and insurance
• Aid members in setting up their health care portal, CEHR, as needed
• Maintain a professional atmosphere
• Complete supply orders for office
• Maintain compliance with filing of clinical information by uploading into PERKS or designated EMR
• Answer phones and email timely complete any scheduling changes as requested by members
• Ensure members are seen for medically necessary services by maintaining schedules for PsyGenics Clinical staff within PERKS including but not limited to the case management team, nurses, psychiatrists, therapists, psychologists, and occupational and speech therapy services
• Check members out of the clinic after services have been provided and ensure that they have the required ongoing appointments for assessment, treatment planning and services set up
• Aid members in rescheduling medically necessary appointments when members call to cancel appointments
• Maintain transportation scheduling and address all related needs
• Complete word processing tasks when directed
• Review daily appointment queue in PERKS and follow up with any members who have cancelled upcoming appointments for immediate reschedule for all services including but not limited to case management
• Ensure all mandated and necessary brochures and literature are well always stocked in the waiting areas/lobbies and provided to members as needed
• Provide and assist in safety protocols and drills
• Maintain the reception and waiting areas
• Must be flexible and possess an ability to quickly adapt to changing policies, procedures and daily tasks
• Possess an ability to identify potential problems/barriers to operations and provide solutions

The Supports Coordinator Assistant will act as an advocate for the consumers and community served and ensure delivery of services.

Specific Responsibilities

• Develops an individualized plan of service based on the comprehensive assessment which includes goals established by all appropriate disciplines
• Conducts self audits as well as peer audits
• Must be knowledgeable in the Mental Health Code and Medicaid guidelines as well as billing protocols and procedures
• Ensure delivery and monitoring of services as prescribed in the Person Centered Plan
• Maintain PsyGenics’ expected productivity level
• Completes and secures required signatures
• Assists Supports Coordinator in the delivery of referrals, services and implementation of the Person-Centered Plan

Position Summary

The Assessment Specialist is responsible for completion of annual clinical assessments and intakes for new cases. Upon assessment, initial referrals are made to all appropriate internal programs. Demonstrated ability to complete all intake documentation thoroughly including but not limited to consents for treatment, releases of information, biopsychosocial assessments and preliminary plans. Timely and skillful handoff to the Case Manager is required to ensure development of a Person-Centered Plan. As a secondary function, this position provides case management coverage to support during a period of staff leave.

Specific Responsibilities

• Provides an overview of PsyGenics and the services offered internally and externally to new members
• Maintains current information regarding both internal and external services and resources
• Explains and obtains consents for treatment, financial determination and other intake documents to members and families
• Completes a thorough biopsychosocial assessment based on sound clinical rationale
• Assists non-master’s level staff with completion of annual biopsychosocial assessments
• Develops a preliminary treatment plan based on the needs identified in the biopsychosocial assessment
• Refers and schedules new intakes with internal services including but not limited to Case Management, Psychiatrist, Link to Wellness, and other appropriate disciplines
• Completes an intended beneficiary survey with new members after the intake process
• Provides resources as appropriate
• Completes all documentation in the Electronic Medical Record system within 24 hours
• Maintains updated calendar in the Electronic Medical Record daily
• Maintains at least 50% productivity (i.e., face to face contact)
• Collaborates with Clinical Supervisor and other team members
• Must be knowledgeable in the Mental Health Code and Medicaid guidelines as well as billing protocols and procedures
• Participates in clinical supervision, team meetings and case consultation
• As appropriate, obtain all prior authorizations and applicable coordination of care
• Provides temporary gap coverage in the absence of primary case holder including completion of treatment planning, coordination of services, and referrals.

Position Summary

The Quality Specialist is responsible for monitoring, evaluating and educating the PsyGenics team to meet contract standards and ensure high quality services are delivered to our members by trained and competent staff. The position also requires providing education and coaching to PsyGenics team members regarding performance expectations for service provision and delivery, safety standards and other training related to best practice.

Specific Responsibilities

• Perform and document quarterly and annual audits as directed by funding source to ensure ongoing conformance to required standards and identification of areas for performance improvement.
  • Includes but is not limited to chart reviews, Medicaid claims, Adverse Benefit Determination audits, program specific reviews, electronic data validation reviews, etc.
• Ensure readiness for regular site reviews by funder including but not limited to Customer Service, Recipient Rights, and Quality teams.
• Complete and submit corrective action plans if required.
• Coordinate with PsyGenics Medical Director to ensure completion of Physician Peer Reviews monthly.
• Oversee incident report module, review incident reports, and report on incidents monthly.
• Review critical and sentinel events and send to case managers and supervisors at least monthly
• Assist with root cause analysis completion
• Track and trend critical events, sentinel events and root cause analyses
• Conduct the Safety Committee Meeting monthly
• Monitor Performance Indicators 2, 3, 4, and 10 and coordinate with leadership and funder as needed for follow up
• Train staff on quality assurance, health and safety, and HIPAA/Privacy standards as needed
• Conduct analysis and develop quality-related reports for leadership including data analysis on all auditing, monitoring, and related quality activities
• Participate in monthly PsyGenics Quality Improvement Committee
• Attend meetings with our funder including but not limited to monthly Quality Operations Meeting, Performance Indicator workgroup, Customer Service Meeting
• Assists in ongoing accreditation activities for CARF compliance

Position Summary

Position Summary

The Psychologist is responsible for psychological evaluations of consumers including administration and the interpretation of psychometric instruments while providing direct services to agency consumers. The psychologist is also responsible for the development of measurable and objective treatment goals and plans in accordance with Person Centered Planning, thorough assessments, individual counseling, parent training, family training, behavior modifications plans, and crisis intervention.  The Psychologist must also participate as an active member of the Behavior Treatment Committee and maintain all requirements of said committee.

Specific Responsibilities

• Administer psychological evaluations of consumers, interpret and create written report
• Provide consultation to staff regarding consumer, treatment plan, environmental considerations as well all applicable considerations
• Provide direct therapeutic counseling and intervention to individuals and families
• Conduct staff in-services specific to consumer served to assigned direct care staff
• Monitor implementation of treatment plan by I-team members, consumer and family
• Obtain, analyze and evaluate significant data to develop service plans and construct programs utilizing a combination of treatment modalities and skills
• Provide support services to individuals and families

Position Summary

The Occupational therapist, in compliance with agency policy and procedures, is responsible for providing direct services to clients diagnosed with a Developmental Disability. Must also complete evaluations, recommendations, as well as develop treatment goals and service plans.  Therapist is also responsible for periodic reviews and all documentation pertinent to discipline as well as requested by the Supports Coordinator.  Also must provide community education, consultation with other interdisciplinary team members and respond to consumer service requests. Regular working hours are to be determined based on the needs of the consumers and of the program and may include some evenings and occasional weekends.  Typical business hours are 8:30am-5:00pm.

Specific Responsibilities

Position Summary

The Case Manager is responsible for ensuring all service needs of a consumer are met by providing linking, coordinating, advocacy, negotiating and referral services.  The Case Manager is responsible for the assurance of a Person Centered Plan as well as the monitoring of the established goals and the implementation of various treatment plans.

Specific Responsibilities

• Develops an individualized plan of service based on the comprehensive assessment which includes goals established by all appropriate disciplines
• Acts as the Chairperson of the multidisciplinary team
• Conducts self audits as well as peer audits
• Must be knowledgeable in the Mental Health Code and Medicaid guidelines as well as billing protocols and procedures
• Coordinates and completes all necessary referrals on behalf of client
• Monitor all services provided on behalf of client
• Monitor satisfaction with services
• Provide resource development in an attempt to reduce the amount of service gaps
• Develop and maintain written records necessary for professional practice as defined in the procedural manual
• Provide outreach services to individuals and their families in their respective locales and/or homes as appropriate to their unit’s function
• Provide community education services as assigned
• Participate in clinical supervision, team meetings and case consultations
