Position Summary
The Assessment Specialist is responsible for completion of annual clinical assessments and intakes for new cases. Upon assessment, initial referrals are made to all appropriate internal programs. Demonstrated ability to complete all intake documentation thoroughly including but not limited to consents for treatment, releases of information, biopsychosocial assessments and preliminary plans. Timely and skillful handoff to the Case Manager is required to ensure development of a Person-Centered Plan. Position Summary
The Occupational therapist, in compliance with agency policy and procedures, is responsible for providing direct services to clients diagnosed with a Developmental Disability. Must also complete evaluations, recommendations, as well as develop treatment goals and service plans. Therapist is also responsible for periodic reviews and all documentation pertinent to discipline as well as requested by the Supports Coordinator. Also must provide community education, consultation with other interdisciplinary team members and respond to consumer service requests. Regular working hours are to be determined based on the needs of the consumers and of the program and may include some evenings and occasional weekends. Typical business hours are 8:30am-5:00pm.

Qualifications

Position Summary
The Case Manager is responsible for ensuring all service needs of a consumer are met by providing linking, coordinating, advocacy, negotiating and referral services. The Case Manager is responsible for the assurance of a Person Centered Plan as well as the monitoring of the established goals and the implementation of various treatment plans.
Specific Responsibilities
*Limited Licensed professionals may also function in this role with the understanding that they must maintain ongoing supervision until such time as they receive full licensure. If appropriately licensed staff are available and able, PsyGenics will assist with supervision. In lieu of in-house supervision, Limited Licensed professionals may also choose to provide documentation of ongoing outside supervision on a quarterly basis until such time as they receive full licensure.
